Electronic Request for Records
Union Health now offers an Online Patient Record Request tool for the following individuals to request patient records:
- Patients requesting their own records
- Patient requesting their own records to be sent to someone else
- Parents of minor patients requesting records
- Caregivers acting on behalf of a patient (i.e. Power of Attorney)
The Online tool verifies your identity by asking for a photo of your driver's license or Government issued identification, which can be taken via webcam or smartphone. There is no additional charge to use this service.
Release of Information
To obtain copies of health information, the hospital must have an authorization form; this form must be signed by the patient or his/her legal representative. A picture ID will be required. Records can be made available electronically, upon request.
