Patient portal proxy Access

Proxy access allows parents, legal guardians, and caregivers to log into their personal portal account and connect to the health information of a patient they represent. While logged in you can communicate with the doctor’s office and enjoy other resources such as managing appointments online and reviewing test results.  If you are a proxy requesting access on behalf of the patient you must either present with the patient, with a valid photo identification, social security card, birth certificate, or other acceptable proof of relationship to the patient. There are two easy ways to obtain Proxy access:

 

1. Download and complete the patient portal consent form.

2. Ask about proxy access at the patient’s next scheduled visit to the doctor’s office.

Patient portal Revocation of proxy access

Portal Revocation of Proxy Access allows you to revoke portal access to your medical records by a previously authorized individual (your “proxy”). You must present this form along with photo identification, social security card, birth certificate, or other acceptable proof of identity to revoke a proxy.
