Successful Completion of the Residency Program Requires:
- Checklist for Mandatory Activities
- Indiana Pharmacist License by August 15th (special exceptions may be granted)
- Failure to obtain licensure by this date may result in a 60-day grace period. Failure to obtain licensure by October 15th will result in termination of the residency agreement
- BLS and ACLS Certification must be completed by the end of Orientation
- Completion of a minimum of three (3) formal presentations
- Completion of a minimum of one (1) MUE
- Completion of a minimum of one (1) drug monograph
- Present a minimum of two (2) journal clubs
- Presentation of residency project at Great Lakes Residency Conference
- Successful completion of Residency Project Manuscript
- Completion of all Teaching Certificate Program related education activities and receipt of certificate
- Successful completion of all learning experience activities as outlined by syllabi
- Completed resident portfolio (stored on pharmacy shared G drive)
- 90% required goals evaluated and scored as “Achieved”
- Residency certificate presentation approved by Residency Advisory Team
Please note the above are minimum requirements for successful completion of the program. Residents will be given ample opportunity to meet the above required activities.
